In answer to your questions ' If a adoption placement breaks down, do the birth parents automatically gets notified. If not, could you explain why' ?

The Service Manager for the Adoption service in North Yorkshire Children and Young People's Service, Children's Social Care has given the following response -

If it breaks down before an order is made, there is a duty to inform those with parental responsibility, which includes birth parents.

If it breaks down after the adoption order, there is no automatic duty to inform. There is discretion to do so if this is felt to be in the child's best interests.
