Acess to my deceased mothers medical records
Hannah Roper made this Freedom of Information request to United Lincolnshire Hospitals NHS Trust
The request was successful.
From: Hannah Roper

Dear United Lincolnshire Hospitals NHS Trust,
I am writing to request information under the Freedom of
Information Act 2000. In order to assist you with this request, I
am outlining my query as specifically as possible.
My mother passed away the day after i was born in 1985 and she was
very ill all throughout her short life. My family have never been
open in discussing medical matters and talking about my mother
upset them greatly. As a result of this I have very little
information in regards to her medical background. Most of my
relatives who could have told me this information have also passed
away now. I would really like to know if any of her medical history
could have an effect on me personally.
Is it at all possible to see these medical records? She was in
Lincoln hospital when she passed away if this is of any help to my
enquiry. Obviously I cant go into specific personal details on a
public website but any information you could give me would be
greatly appreciated.
Yours faithfully,
Hannah Roper

I simply want to know if this is possible. And if it is, who do i need to contact to make this happen!

Hannah, You need to place a Subject Access Request, You'll need to provide id and maybe pay (up to £10). Some orgs charge, others don't. There may be a form to fill out. Try having a look on their website.

Dear Hannah
Sincere apologies for the delay in responding to your request.
It is necessary to complete a form to gain access to deceased medical records and this can be found on our website at the following link.

I hope that this helps and once again apologies for the delay.
