Dear Bradford Teaching Hospitals NHS Foundation Trust,

Under the Freedom of Information Act 2000, I would like you to
supply documentary evidence for each of the following:

Which individuals/ departments/ subsidiary organisations either
employed by, sub-contracted by, self employed or through
association with your organisation or with any form of affiliation
to your organisation have access to medical patient care and
summary records, both paper and electronic, that are administered
within the your district. This is in addition to records held and
administered by patients' General Practitioner or persons that may
be employed or access data at a General Practitioners surgery.
Which individuals/ departments/ subsidiary organisations either
employed by, sub-contracted by, self employed or through
association with your organisation or with any form of affiliation
to your organisation, excluding General Practitioners or persons
that may be employed or access data at a General Practitioners
surgery, have access to infant inoculation records besides a
patients' GP?

Is a back-up system or system employed within your district where
duplicates of any or all patients records, both current and archive
to include former paper records, are stored either as paper or in
electronic format? If so, where are these records stored, what
records are stored and which individuals/ departments/ subsidiary
organisations either employed by, sub-contracted by, self employed
or through association with your organisation or with any form of
affiliation to your organisation, excluding General Practitioners
or persons that may be employed or access data at a General
Practitioners surgery, have access to such records?

Which databases exist, within the district and nationally, exist to
provide individuals/ departments/ subsidiary organisations either
employed by, sub-contracted by, self employed or through
association with your organisation or with any form of affiliation
to your organisation with access to any or all patient records? Can
any or all of these databases be accessed using an individual's NHS
'smart card' and if so, which individuals/ departments/ subsidiary
organisations either employed by, sub-contracted by, self employed
or through association with your organisation or with any form of
affiliation to your organisation have access to which databases and
where are these individuals/ departments/ subsidiary organisations
either employed by, sub-contracted by, self employed or through
association with your organisation or with any form of affiliation
to your organisation located?

Which individuals/ departments/ subsidiary organisations either
employed by, sub-contracted by, self employed or through
association with your organisation or with any form of affiliation
to your organisation enter parent or child information into the
ContactPoint database and which individuals/ departments/
subsidiary organisations maintain the accuracy of the database and
how is this performed?

How do individuals/ departments/ subsidiary organisations either
employed by, sub-contracted by, self employed or through
association with your organisation or with any form of affiliation
to your organisation access ContactPoint and to what data specifics
do they have access to?

When compiling a response to this request, should the law prevent
your organisation from identifying any individual by name then
please state their individual or employed title as an alternative.
