Dear Sir or Madam, I was on an A4e course in southport merseyside,
from october 2008 to 29.3.2009. in october i got a temp job while
at a4e i had to sign a form about my job, a4e sent the form to job
center plus southport, i would like to see it. when my time at a4e
ended a4e gave me a form to fill in then sent it to southport job
center i would like to see it. I made a complaint to southport job
center about a4e southport. tricia barton from 3rd party provision
team cressington house 249 st marys road liverpool l19 onf, dealt
with the complaint. I would like to see any information about the
complaint that they have, and any information southport job center
have on the complaint.

[you state;Your request for copies of personal information held on

possession. no you never sent the form for when i got my job while

As your request refers to the disclosure of personal information this would be covered by guidance obtained from the data protection act 1988.
I have checked and reviewed your original request and after futher investigation i can confirm that all available information was sent to you in our original reply of 27 october.

district office sent you a proforma on 09 July 2009. that is
correct i filled it in and sent it back. you say,,
The Data Protection Officer has advised that they wrote to you
again on 30 December 2009 enclosing another copy of the proforma
which requested further information from you to ensure that we can
correctly identify you and locate your records.(I never got it can
you send it me again)so you are saying chester office wrote to me

request can you not see how daft you look? to all you who are
