     Can the PCT release a copy of its current policy for
     assessment/treatment for gender reassignment/dysphoria?

     What is the officially recognised path for patients requesting
     gender reassignment?

     Is gender dysphoria classed as a mental-health issue by
     Nottinghamshire County Teaching PCT?

     How does the PCT deal with GP's formally requesting psychiatric
     referral for diagnosis of gender dysphoria?

     Is there a waiting list for patients requesting 'Gender Identity
     Clinic' referral, how long is the average wait?
