Dear Mr Clark

Request for information under the Freedom of Information Act 2000

Thank you for your request for information about:

Chemotherapy induced febrile neutropenia

As you may know, every time a patient has an episode of care, this is coded
in terms of their diagnosis, treatment and outcome.  This information is
anonymous but allows for statistics to be collated.  I understand that the
clinical code for neutropenia is D70 and this includes drug-induced.  There are
more detailed secondary codes that are used and codes from Y40.0 to Y59.9
would include drugs, medicaments & biological substances causing adverse
effects in therapeutic use.  Therefore our Information department have
searched for cases with these codes and then looked at the outcome.

I enclose a spreadsheet with this information.

I trust that I have dealt with your request to your satisfaction.  Please do not
hesitate to contact me if you have any further queries or if I can be of further
assistance.

This information has been held by the Trust for its own purposes and is
published to you pursuant to the Trust’s obligations under the Freedom of
Information Act 2000.  Any further use or publication by you of this material is
at your own risk and it is your responsibility to check the accuracy and
appropriateness of the material for your purposes.

Finally, I enclose an evaluation form which I would be very grateful if you
could complete and return to me so that we can continually improve our

Publication Scheme and the way that we deal with requests for information
under the Freedom of Information Act.

Yours sincerely
