Dear Sir/ Madam
E mail: [email address]
I am writing regarding your request for information, which I received on 15 January
2009. In that request you asked us for the number of people who were diagnosed
with febrile neutrophenia and the number of deaths for 2008.
Cumbria Teaching Primary Care Trust reasonably requires further information in
order to identify and locate the information that you have asked for. We require the
ICD10 code that you wish us to use to enable a search to be undertaken in order
that we can extract the correct information for you. I will not be able to take this
matter further without extra information from you.
If you wish to discuss any of the above, please contact me. Please remember to
quote the reference number above in any future communications.
If you are unhappy with the service you have received in relation to your request and
wish to make a complaint or request a review of our decision you should write to:
Complaints & Projects Manager
Cumbria Teaching Primary Care Trust

If you are not content with the outcome of the internal review, you have a right under
section 50 of the Freedom of Information Act 2000 to apply for a decision by the
Information Commissioner as to whether we have properly dealt with your request.
Please note that an application to the Information Commissioner may be refused if
you have not exhausted any complaints procedure we provide, if there has been
undue delay in making it, if the Information Commissioner considers your application
is frivolous or vexatious or if your application has been withdrawn or abandoned.
