Dear Mr Kumar

Request under Freedom of Information Act 2000

Our Reference: 298-MK-GP Retirement

Thank you for your request for information regarding GP retirement, which I received on 19th May.

There has been no GP's who have retired but continued to practice since 2004. Which means there has been no entry on performers list removed and then reapplied.

If I can be of any further assistance then please do not hesitate to contact me. If you are unhappy with the way the PCT have handled your request then you can ask for an internal review. Please contact the Patient Advice and Liaison Service (PALS), quoting the reference above. xxxx@xxxxxxxxxxxxx.xxx.xx

Yours sincerely
