I am writing to remind you that your contract for [insert name and address of premises] expires on [insert date].

I trust that you have found the service we provide satisfactory. If you wish your contract to be renewed for a further period of twelve months the contract price will be £ including VAT. This is to allow for a maximum of [insert No] visits to your site.

I should be pleased if you would complete one copy of the enclosed contract form and return it, along with a cheque for the above sum made payable to Scarborough Borough Council, to this office. A second copy of the form is enclosed for your records. The price will remain as stated for one month from the date of this letter.

PEST CONTROL CONTRACT NUMBER:

(hereinafter called “the occupier”)

It is hereby agreed as follows:-

1. The occupier authorises the Council's officers to enter upon the premises known as [insert name and address of premises] and undertake treatment for the control of rodents during the period of twelve months commencing [insert date].

2. The occupier agrees to make all necessary arrangements free of charge to enable the Council's officers to begin the works contracted for without hindrance or delay and to give such information as may be required of them.

3. The occupier agrees that poison bait may be left on their premises and will not interfere with such bait but will take all necessary precautions to prevent all persons, children, domestic animals and livestock from obtaining access to it and while Scarborough Borough Council will exercise all reasonable care in the execution of the work, they will not be liable for any loss or damage whatsoever arising as a result of the treatment unless such loss or damage be as a direct result of the negligence of the Council or their officer.

4. The occupier agrees to indemnify the Council against any or all costs, claims or actions which may be made against them by any third party in respect of any injury, loss or damage arising as a result of the treatment and agrees to reimburse the Council for any loss or damage to its equipment.

5. Whilst advice may be given by the Council's officers the Council shall have no responsibility for any works or proofing the premises against re-infestation and whilst every effort will be made to eliminate the above pests, no guarantee can be given against re-infestation.

6. The charge for the work shall be £[insert amount] including VAT at 17.5% and I agree to pay the said charge to the Council in advance of the contract agreement. Cheques should be made payable to 'Scarborough Borough Council' and sent with this form duly signed to the above address. The Council reserves the right to review the charges as and when considered necessary.
