Enclose your cheque or postal/money order for payment (£30.00 sterling payable to MOD Accounting Officer) if you are not the widow or widower of the former service person.
Enclose the fully completed Certificate of Kinship.
Enclose a death certificate or proof of death (obituary notice, funeral certificate etc) if the person died after leaving the service. You may be able to request a copy of the death certificate from The Registrar of Births Marriages and Deaths.
