I am looking to compile a list of the contact details of NHS authorities in Wales including:-

1) their official name, full address and postcode and;

2) their Information Security Officer or IT Security Manager name and e-mail address,

3) their Freedom of Information officer name and e-mail address,

4) their Data Protection Officer name and e-mail address,

The National Assembly for Wales does not hold the information you have requested.

The HOWIS website (Health of Wales Information Service) provides an NHS Wales Directory which you may find useful http://www.wales.nhs.uk/directory.cfm

You may wish to contact Local Health Boards individually requesting the specific information that you have requested.
