Dear D.Speers,

Your application under the Freedom of Information Act for information on investigation data

I am writing in response to your Freedom of Information Act request received on the 4 December
2009.
Our Trust has never been the subject of an investigation by:
     1) the Commission for Health Improvement (CHI)
     2) the Healthcare Commission (HCC)
     3) the Care Quality Commission (CQC)

There are currently no secondments of staff at (grade 8 or above) between our organisation and our
strategic health authority and there are no records of any such secondments in 2007-8 or 2008-9.
We would have to consult our manual records for any data earlier than this, which would cost more
than £450 in staff time, which is greater than the ‘appropriate limit’.

I hope that you find this satisfactory.

The Trust provides a complaints procedure for the Freedom of Information Act and if you are not
satisfied with the response, you should write to the Chief Executive.  Furthermore, you have a right to
complain to the Information Commissioner for a determination under Section 50 on whether the Act
has been complied with.  The Information Commissioner’s details are: Information Commissioners
Office, Wycliffe House, Water Lane, Wilmslow, Cheshire, SK9 5AF.

Yours sincerely

Mr ‘Jide Odusina
