With apologies for the delay in responding, I am writing to you further to your 
request for information about: 
 
 
Investigations by CHI, HCC and CQC and secondments to Strategic 
Health Authority in the past three years 
 
The Trust has examined the records it holds and I can confirm the following 
answers to your questions. 
 
The Trust was issued with an Improvement Notice concerning the Hygiene 
Code by the Healthcare Commission in July 2007 which was lifted in 
December 2007.  However, the Notice did not result from an investigation, but 
from a routine inspection.  The Trust has not been subject to any 
investigations by the Healthcare Commission or Care Quality Commission 
other than the routine regulatory reporting and review requirements. 
 
The Trust has not seconded any staff to the Strategic Health Authority.
