With the exception of Question 4, answered below, your queries all seem to refer to general medical theory and as such are exempted under Section 21 of the Freedom of Information Act 2000 as the information requested is widely available by various other means. The answer to question 4 of your enquiry is as follows:

Papworth Hospital NHS Foundation Trust uses biological heart valves made by Carpentier-Edwards, St Jude Medical, Sorin, Medtronic and Vascutek Ltd, and mechanical valves made by Carpentier-Edwards, St Jude Medical, and Calmed Ltd.
