I would be grateful if you could provide me with figures at UHB
Foundation NHS Trust for the year 2007 to 2008 of:-
1. the number of wrong site operations
2. surgical instruments left in situ following surgery
3. operations cancelled due to incorrect documentation
4. surgical infection figures
5. surgery related injuries or fatalities
