Re: Request under Freedom of Information Act 2000 - Drugs Formulary Committee
Thank you for your request for information regarding the above which I received on the 20th August 2008.
Due to the merger of Swansea NHS Trust and Bro Morgannwg Trust on the 1st April 2008 to establish the ABM University NHS Trust, a new formulary committee for the new organisation is still in the process of being set up. We hope to have full membership details by the beginning of October, therefore if you wish to contact us again at this time, please do so.
The Trust operates a system of Internal Review. Should you consider that our response to your request has not been managed appropriately please contact my office at Trust Headquarters below.
If after Internal Review you remain dissatisfied you are also entitled to refer the matter to the Information Commissioner at the Information Commissioner's Office, Wycliffe House, Water Lane, Wilmslow, Cheshire, SK9 5AF (Tel 01625 545 745, Fax 01625 524510).
