Legal advice sought and gained to ensure that Barnet’s forms for a 
fiver scheme is lawful and compliant 
 
Although Barnet and Chase Farm Hospitals NHS Trust has agreed that it 
wishes to pursue an application for authorisation as a Foundation Trust, it has 
not embarked on the formal application process as yet and is not actively 
recruiting members.  Therefore it does not have a ‘forms for a fiver scheme’ 
and does not hold the information you have requested.
