Guidance Notes

This form helps managers to record their assessment of an applicant's suitability for the job in a structured way.

·         The chair of the interview panel is responsible for completing a Short-List Form to record the decisions of the short-listing panel. Each manager involved in the short-listing process may complete their own copy of the short-listing form as part of their preparations, however a final version should be agreed and will be keep as a record of the decisions made.

·         Each applicant's name should be entered in column one. This must be done for all applicants for the post.

·         Using the person specification each applicant should be assessed to confirm whether they meet the essential and desirable criteria for the job. A tick should be placed in each numbered column to confirm they have met this criteria. A cross should be used to indicate the applicant has not provided evidence to meet the selection criteria. Where it is not possible to assess a selection criteria at this stage a question mark should be placed in the column.

·         The selection criteria numbers in column 2 should correspond to the selection criteria on the person specification i.e. the first essential criteria would be number 1, the second number 2 and so on.

·         The comments column can be used to note any issues which make the applicant unsuitable and to record the short-listing decision i.e. `yes' if the applicant is to be short-listed and `no' if the applicant is not to be short-listed.

·         Once completed the short-list form should be return to the EAS team, along with all the application forms (both successful and unsuccessful).
