Re: Request under Freedom of Information Act 2000 - Medicines Formulary

Thank you for your request for information regarding the above which I received on the 21^st July 2008.

You will wish to know that Swansea NHS Trust merged with Bro Morgannwg NHS Trust on 1^st April 2008 to establish the Abertawe Bro Morgannwg University NHS Trust.

I have attached the most up date to version of the formulary for ABM University NHS Trust for your information. I would however draw to your attention that this information has been provided for your own private use and not for any further commercial purposes. Under the Re-Use of Public Sector Information Regulations 2004 you are not therefore automatically entitled to re-use the information.

The Trust operates a system of Internal Review.  Should you consider that our response to your request has not been managed appropriately please contact my office at Trust Headquarters below.
